    List an additional two persons authorized to pick up your child from camp.

    Authorization Information

    In the event that my child requires medical attention, I give permission to the zoo camp staff at reptiland to treat injuries that require basic first aid. In the event that my child requires medical treatment for severe injury, I give reptiland permission to call emergency services. Reptiland will attmept to contact me at the number listed below.

    1. By signing and submitting this PARTICIPATION / REGISTRATION FORM you agree that you have read and understand the Policies as outlined in the POLICIES & PARTICIPATION OUTLINE and agree to comply with the rules and conditions stated within.

    2. By signing and submitting the ZOO CAMP 2023 EMERGENCY CONTACT INFORMATION FORM, you agree that you have provided all emergency contact information and have read and understand the Medical Authorization Form as outlined within the form.

    3. By signing and submitting the REPTILAND GRANT OF PERMISSION /GRANT OF USE/RELEASE & HOLD HARMLESS AGREEMENT, you agree that you have read and understand all information as stated within the form.
